Intervention Details


EpxFallRisk is an interactive telephone system that provides a simple method for older adults to prospectively keep record of their fall history and communicate it with their healthcare provider.

Patients 65 years or older.

Communication Modes
Patients may receive messages once, daily, weekly or at customized intervals designated by their healthcare provider.
Default Messages
  • How many times have you fallen since 7 days ago? Please use the keypad to enter the number, and then press pound. If you did not have any falls, please press 0.

Patient reported falls or stumbles are alerted to provider via phone call and email.


Patients will be directed to call 911 if they are experiencing an emergency. You may designate a care provider to be acutely notified in the event of a defined critical value.
